Established in 2025, Taconic Essential Primary Care, PLLC was founded in Rutland, Vermont, by a mother–son team: Tammy S. Devereux, APRN, and Matthew Dayton, CCMA. Their vision was simple yet powerful: to create a practice where patients feel cared for as individuals, not just as medical cases.

​With more than 35 years of nursing experience, Tammy has dedicated her career to serving patients with skill, compassion, and integrity. Her background across diverse clinical settings shaped her belief that primary care should prioritize listening, trust, and lasting provider–patient relationships.

Together, Tammy and Matthew bring a unique balance of clinical expertise and personal connection. Their family-run practice represents a new chapter in local healthcare—one that blends professionalism with the warmth of a small, community-focused office.

Meet Our Office Manager & Co-Owner
Matthew Dayton, CCMA

Matthew’s passion for healthcare began at a young age, when he often accompanied his mother to the nursing home where she worked. Even then, his caring nature was evident—he could be found sitting with residents, helping to feed them and offering companionship. At 16, Matthew took his first formal role in healthcare as a kitchen aide, and in 2012 he earned his Licensed Nursing Assistant (LNA) certification.​
​
In 2020, Matthew further advanced his career by completing his Medical Assisting Certificate, expanding his skills to better serve patients in a variety of clinical settings. His hands-on experience and genuine compassion for others have shaped his approach to patient care, focusing on respect, dignity, and personalized attention.

In 2025, Matthew partnered with Tammy to establish Taconic Essential Primary Care PLLC, with the shared mission of providing high-quality, compassionate primary and preventive care. Together, they emphasize patient-centered healthcare while offering a wide range of services, including wound care, DOT physicals, preoperative exams, school physicals, lab testing, and telehealth visits.
