Meet Our Provider & Co-Owner
​​Tammy Devereux, MSN, APRN, AGNP-C

A lifelong Vermont resident, Tammy understands the unique healthcare needs of her community. She is dedicated to providing patient-centered primary care that is non-judgmental and empowering—helping patients take an active role in their health and well-being.
With over 35 years of healthcare experience, Tammy brings both depth and compassion to her role as a primary care provider and co-owner. Her journey began in 1991 as a Licensed Nursing Assistant, continued with her Licensed Practical Nurse (LPN) license in 2000, and advanced with an Associate of Science in Nursing in 2010.

Tammy’s expertise spans a broad range of Vermont healthcare services, including inpatient and intensive cardiac care, medical intensive care, and palliative care. As a trusted nurse practitioner in Vermont, she provides adult primary care, diabetes counseling, weight management, women’s health services, as well as home care and assisted living support. Her well-rounded approach ensures patients receive compassionate, individualized care at every stage of life.
